You will need to apply for a duplicate card if:

• you lose your card or if there is a change in any of the information printed on it (name of company, ID number). You should apply for a new card using the following form (price according to the current price list)

You will need to report a change of information if:

• there is a change in any information not printed on the card (address, contact details). Just let us know by the following form
