Duplicate card/change of information
You will need to apply for a duplicate card if:
- you lose your card or if there is a change in any of the information printed on it (name of company, ID number). You should apply for a new card using the form “Application to block/unblock, application for a duplicate, application to change information” (price according to the current price list)
You will need to report a change of information if:
- there is a change in any information not printed on the card (address, contact details). Just let us know by submitting the same form containing the correct current information
