To get a Company Pilsen Card, go to:

• the PMDP Customer Centre at Denisovo nábřeží 12 or Klatovská třída 12
• or, after making an appointment at infolinkapk@pmdp.cz, in the Card Centre at Divadelní 3.

and submit the following documents:

• a valid identity card for the person given as the contact person in the application,
• where applicable, a legalised power of attorney for the person who is to act as contact person for the company,
• if you want your company logo printed on the card, you will need to provide it in electronic form (jpg format).
